Had my car pushed off the side of a cliff by a Roller-Mine.

 

Modern Warfare 2: Got killed in last kill by a bouncing throwing knife that bounced off the ground and wall then killed me.

 

Super Mario Galaxy: I fell off a planet. Lame? I know.

 

Super Smash Bros. Brawl: I got pooped out of a Yoshi.

I was in Hardcore Ricochet Search and Destroy about 5 minutes ago. I threw a Semtex over a wall to kill an enemy that I saw. Apparently, my team mate killed the guy and my Semtex ended up hitting my team mate. It blew up, and since friendly fire is directed back at me, I died.

Super Smash Bros. Brawl: I was playing a Stamina match, Toon link (me) versus the Ice Climbers on Final Destination. The Ice Climbers were hanging off the side of the arena, and I saw my chance to end the match. I did a midair sword plant... but I was a bit too far to the left. Boom, instant KO, You Lose.

Got smacked off the path in my first fight against a Giant Depraved One in Demon's Souls.

 

I lost my copy of Spirit Tracks. It was turning out to be an amazing game, too D:
