KH-Vids Yearbook: Signing up

So we're gonna make a Yearbook for our great site. To appear in the yearbook all you need to do is fill in this template:

Name:
Picture:
Caption: ​

If you want to have your username and then aka another name you like being called that's fine (For example: Ninja24 aka John).
Picture doesn't have to be a personal one of yourself but that atleast somewhat represents you. Please, if possible, give me an image that is not more than 300x one way lengthwise nor heightwise.
Caption: Short four-five sentence caption about you. You can pretty much write what you want here.

For any other info, ask here or PM DA, Misty or me.
