It's where you put the digits listed in the category you grabbed that code from. Ex: "0003 = Item"
You'll have no luck with that, even if you do. The Drive Form abilities are just flavor text that don't actually add anything to the moveset.
Have you tried not activating it until you get to the room? EDIT: If you're using Crazycatz00's patch, there's a chance it will not work. That one changes UCM locations.

That didn't work for me, but I was using an original Japanese ISO. You should probably edit that message again and specify your patch.
Off the top of my head, there are three things that could be causing your problem. 1: You made those moveset modifications, then reloaded the battle with a mob added or removed that wasn't before (ex: You made a file dump, created a moveset mod, then decided afterwards that you wanted to remove Donald). If this is the case, scrap everything you did and redo your file dump when you are sure your conditions are perfectly set up. Conditions include role mods (ex: changing a mob to a partner/enemy/neutral role) and what mobs are loaded into the battlefield. 2. Saix is a special boss in that he spawns extra mobs, since his claymores that give you a Reaction Command are actually treated as boss roles. If you made this file dump after he threw out a claymore, it could have possibly interfered. If this is the case, scrap what you did and make the file dump as soon as he starts attacking, but before he lets go of a claymore. 3. You put something into your data that was an incorrect value (ex: maybe you accidentally made 56B's animation = 56C's effect, or you made 56B's animation = 56C). I suggest a bit of trial and error where you remove one line at a time (keep it saved in a text document or whatever you use to store data) and see if a crash still occurs. If this doesn't work, only apply a single line of code from your moveset mod at a time as a continued form of testing. If that doesn't work, your problem is probably one of the previously mentioned things in this list.
Assemble a slot 1 UCM code using the "Universal Character Modifier" section of the front page. You can find the digits, on your own, using the link at the beginning of the section.
Summon modding is one of those cases where you have to do a MDLX/MSET swap. Wheel of Time wrote me this tutorial a long while back, but I'll modify it a bit for your case: 1. Create a file dump of KH2FM using Emuhaste. 2. Open the dump in ps2dis. 3. Press Ctrl+G to open up the label searcher and search for a model name you want to replace. In your case you want to search for a summon model, so let's go with Chicken Little. He has the model name P_EX350. Type that in with quotation marks around it and open the first result. 4. Copy the address lines where the label stretches which is two lines in this case, might be bigger than two lines but you'll notice it (like in this case, 01caeed0 and 01caeed4) For reference, the last digit in your address code should always be 0, 4, 8, or c. Do this for the mdlx and the mset. 5. The value you see after the addresses (data) is what we should replace. 6. Search for another character's model, like the Cloud partner (N_HB550_BTL) and copy the data instead of the addresses. 7. As you can see this model has three lines instead of two, so we have to add another line to our addresses which is the next address after the ones we already have. In this case, 01caeed8. 8. The final step is to change the first "0" in the address with a "2" You should now have a code to summon Cloud instead of Chicken Little. If you need to find a mdlx/mset name, do this: 1. Press G in ps2dis and go to "01c00000" 2. Type in the UCM (Universal Character Modifier) digit of you want to find. Chicken Little's UCM is 06C8, so search that. 3. You should appear at a location that shows his mdlx and mset name. This also serves as an alternative method for step 3 in making your mdlx/mset swap. Here are some things to note: There is a list of almost every UCM code on the front page. Go into the start menu of the game before attempting to summon anything, and switch "Summon Effects" to "None." I made the above code on a clean Japanese ISO of the game, and I'm unsure if the addresses are different on patches. If they are, just follow the steps and ignore the reference code I made.

How are you starting KH2FM at all, then? If you burned it to a blank disc, that might effect the use of codes, although I'm unsure.
Are you using a non-Japanese PS2 with Swap Magic?
Use them in worlds that aren't Timeless River, Pride Lands, Halloween Town, or Space Paranoids. Also, Guest slot replacement doesn't work well at all, just stick to Donald and Goofy.
