Eva sighed. She wondered vaguely if she had made a mistake coming here. He didn't need her....but she would see this through, for now. She had no other attachments to him. Steadying her horse, she kneeled down beside him.

I'm pretty sure either Roxas injured it, as stated above, or that he hurt it fighting as Ansem sometime. Since it wasn't there in KHI, he could just as easily hurt it in Chain of memories too.
